Annual Membership Instructions

Welcome to our annual membership renewal process! Please read the instructions below carefully to ensure a smooth renewal of your membership.



For New Members

We are thrilled to welcome you to our organization! To complete your membership application, please follow these steps:

  1. Download the New Member Application: Click here to download the form.
  2. Complete the Application: Fill out all required fields accurately and completely. Leave the ID number line blank.
  3. Provide Oath of Office: Include a copy of your Oath of Office with your completed application.
  4. Calculate and Submit Dues: Please include your payment with your application. Make checks payable to MCVFPA.
    • New Member fee - $3
    • Annual Dues - $5
    • Death benefit (optional) - $2
  5. Return All Documents: Mail your completed New Member Application, a copy of your Oath of Office, and your dues payment to:

For Current Members

Thank you for your continued commitment to our organization! Your personalized membership renewal application will be mailed to you shortly. Please follow these instructions to renew your membership:

  1. Review Your Application: Upon receiving your application via mail, carefully review all pre-filled information. If needed, you can obtain a blank copy of the renewal form by clicking here.
  2. Make Necessary Changes: If any information is incorrect or outdated, please make the necessary changes directly on the form.
  3. Sign and Date: Sign and date the application form in the designated areas.
  4. Calculate and Submit Dues: Please include your payment with your application. Make checks payable to MCVFPA.
    • Annual Dues - $5
    • Death benefit (optional) - $2
  5. Return All Documents: Mail your signed and dated application form and your dues payment to:

For Life Members

We appreciate your dedicated service as a Life Member! While you are no longer required to pay annual dues, we still need you to confirm your information.

  1. Review Your Application: Upon receiving your application via mail, carefully review all pre-filled information. If needed, you can obtain a blank copy of the renewal form by clicking here.
  2. Make Necessary Changes: If any information is incorrect or outdated, please make the necessary changes directly on the form.
  3. Sign and Date: Sign and date the application form in the designated areas.
  4. Death Benefit Payment (If Applicable): If you have elected to participate in the death benefit program, please include $2 payment for the death benefit only. Make checks payable to MCVFPA.
  5. Return All Documents: Mail your signed and dated application form, along with any death benefit payment (if applicable), to:

Change of Beneficiary Form

You only need to complete this form if you wish to change your designated beneficiary at any point after you submit your application. If you are renewing your membership, you may change your beneficiary information directly on the renewal form. This form can be submitted at any point before the start of the next year's membership renewal process.
