New Membership Information
Instructions for new membership:
Complete the New Member Application Form below. Don’t forget to attach a copy of your Oath of Office from your municipality.
When you submit the form, you will be taken to PayPal to pay online. If you prefer to pay by check, mail the check to the address before. If you could not attach a copy of your Oath, please send a copy to that address.
Note: if you prefer to mail the form, download and print the Application Form from the bottom of this page. Complete and mail the form, the appropriate fees and your Oath of Office to the address on the form.

After approval by MCVFPA your membership card and State Fire Police Association application will be mailed to you.
